Note: Within any calendar year, cumulative course accreditation fees shall not exceed $5,000 for an individual training provider.
Lead-Based Paint Activity Notification Forms and Permit Application [PDF]
The notification forms must be completed and submitted on all projects established prior to 1978. For projects requiring lead permits, utilize the permit application forms.
Fees for Lead-Based Paint Hazard Reduction and Control Permits are $40 plus 3% of the abatement cost.
